For the purpose of tracking the number of guests, all delegates are requested to pre-register for the welcome reception and gala dinner when registering for the conference.
CBMTG Allied Health Members receive a discounted rate to the meeting. Please note that there is no discounted rate for non-member allied health individuals; these individuals would register at the regular non-member rate.
Non-members can join CBMTG and then register for the conference at member rates. To apply for membership, complete and submit a membership application form at www.cbmtg.org or phone 604.874.4944.
Registration Deadlines: To qualify for the discounted registration fee, a complete registration form and payment must be postmarked or received on or before February 15, 2012.
Member Registration Fee: If you are not already a CBMTG member and qualify for membership, you are welcome to join CBMTG for 2012 to take advantage of the reduced member fee for the conference. Please note that all CBMTG memberships run from January 1 to December 31, and no pro-rated memberships are available. Memberships purchased at the time of conference registration will expire December 31, 2012. To ensure that you receive maximum benefit from your CBMTG membership, we encourage you to join/renew your membership as early as possible in the calendar year. Membership forms can be downloaded from the CBMTG website at www.cbmtg.org.
Cancellation Policy: For registration fees to be refunded, a written notice of cancellation must be received at the CBMTG Head Office by mail, fax or e-mail no later than March 7, 2012. The amount of the registration fees remitted, minus a $75 administrative charge, will be refunded after the conference. Registration fees will not be refunded for any reason after March 7, 2012. Substitutions are accepted. Please send written notice of substitutions by March 26, 2012.
Conference Meals: Included in your registration fee are the conference Welcome Reception, Corporate Symposia meal functions and coffee breaks.
Languages: All presentations will be in English
Accommodation: For the convenience of conference attendees, a limited number of rooms have been reserved at the Westin Harbour Castle (1.416.869.1600) starting at a rate of $189 per night, plus applicable taxes. Please call the hotel directly to make reservations and identify yourself as being with "CBMTG 2012" to obtain the conference rate. The conference rate may not be guaranteed for reservations made after March 10, 2012, 5pm ET or outside the conference dates.
Special Requirements: If you require special arrangements in order to participate in conference activities, please send a written description of your needs along with your registration form.
